Antonio is becoming more stressed as he is due to have an Individual performance review (IPR) at work. During this, he may have to disclose his diagnosis to his manager to explain the change in his presentation. Disclosing personal information is not something that he feels comfortable doing. Antonio has generally managed very well at work but displays different behaviour at home. He has become more isolated in the home environment and his obsessive behaviour is causing him to lock himself away in the bathroom or bedroom for many hours in the evening, instead of spending time with his family. Because of his obsessive thoughts and behaviour, he has become exhausted and is finding it difficult to concentrate. Stage 2 Antonio has visited his GP to discuss his increasing concerns. His mental state has further deteriorated, and he is now unable to sleep or relax and is becoming more and more distressed about family and work life. He is also experiencing feelings of guilt and is becoming irritable at times. The GP requests the community mental health team to visit Antonio to complete a mental health assessment and to offer the appropriate support that he may need. Stage 3 The community mental health team have attended to complete an assessment with Antonio. During the assessment, Anton discloses that he has started to experience thoughts of selfharm and suicide which are becoming more irrational.
