ase Study Kim has been a patient at your pharmacy since moving to the area approximately 2 years ago when she bought her first home.  She is 28 years old, collects her prescription for Cerazette every 3 months or so and between times, pops in to buy vitamins and sundries and generally have a chat with the staff in the store.  She has called in today and asked for some advice from the pharmacist; as you approach the counter to speak to her, you notice that she seems quite pale and is heaving.  You ask a member of staff to get Kim a drink of water and offer her a seat in the consultation room where you begin to get some more information from her. Kim tells you the reason she has come in today is because she is suffering from a really severe headache and what she has tried so far has provided no relief. In fact, she tells you that this is not the first time she has suffered with a headache like this, although this is the worst one yet. Kim works for the local council in the human resources department and works most of the day at a computer and it is usually when she is at the computer screen that the headache starts; she knows when it is coming on as her vision goes “wobbly” and the office lights make her squint her eyes.  She recently received a promotion which gave her greater responsibility and a pay rise which she was initially grateful for as she was finding it tough to make ends meet.  However, the council have had to implement a second round of imposed government budget cuts and people are currently being re-interviewed for their posts, which Kim is responsible for along with redundancy notification.  She is finding it very stressful as some of these people were her colleagues before the promotion and they have fallen out with her over the situation.  She now wishes that she had never taken the promotion but feels unable to step down due to the loss of earning and also that she herself may be in line for redundancy if she does.   As soon as the headache started, Kim took some co-codamol tablets she had previously bought when she sprained her ankle at the gym; past experience told her that paracetamol just wouldn’t do it and although she feels a little sluggish after taking them, it has always done the trick along with a lie down in a darkened room when she gets home.  While it provided some relief for a couple of hours, the pain came back and got worse through the day.  She also noticed that this time she has been very nauseous and heaving quite a lot and although she had felt nauseous with the last few headaches, they had never made her quite so bad.  So much so, that when a colleague who also suffers from bad headaches gave Kim one of her tablets, called Imigran, she vomited it back up within a couple of minutes and it has been all she can do not to be sick again since then.
