ATTN: Dr. Anderson Phipps, MD, Family Medicine/Geriatrics
6249 Broad Street Lebanon, TN 37106
Patient Name:
Doris Elizabeth Moore.
DOB:
07/10/--
Doris Elizabeth Moore, 570 years old, returned and followed up to my office
on August 18. As you may or may not know, she was hospitalized for several
weeks at our Lady of Lords after returning from Paris. She initially presented
with Numanitis. She had been hospitalized on a couple of occasions at Lords,
Kama, and it was felt that this possibly was related to her affinity or. She was
read-mitted to Lords and treated for a bacterial Numanitis with antibiotics. I
was consulted during that hospitalization.
It was felt that possibly she could be having a delayed finitory action, and we
placed her on steroids as well.
She was seen in consultation by pulmonary services and had a bronchoscopy
done.
Her pathology was negative for lymphingidic spread of her tumor, as there
was some suggestion of this by her CT scan. Cultures were negative as well.
While hospitalized, the patient also developed headaches, and a CT scan and
MRI scan of her brain revealed the presence of brain metastases. She was
started on radio to therapy to her brain. Her condition initially declined, and
hospice was considered with supportive care; However, her condition did
improve somewhat, and she was discharged home.
Ms. Moore has now been home a week, and despite some continued fatigue,
she actually looks a fair amount better. On exam, her lungs are clear today.
Her heart is regular. She has trace of Dima.
I had a long discussion with the patient and her family regarding other
therapies. I think the affinity to her played a large role in her pulmonary
problems, and these appeared to have improved somewhat. She continues
to be treated for her brain metastases with radiation therapy.
I discussed other systemic treatments, and we are going to give her a trial of
next of our period. She understands that her condition is quite tenuous,
semicolon. However, we will at least give her an opportunity for further
treatment.
They are referring the patient and her family to Dr. Cynthia Gonzalez for
counseling, period,
Again, I appreciate the opportunity to participate in Ms. Moore's care. Please
do not hesitate to call if I can offer further diagnostic or therapeutic
evaluations,
Sincerely,
time you warn MD, hematology oncology, copy, Cynthia, C, Gonzalez, MD,
