1. The definition of Excellence in Patient Care is “when every patient care act is the right thing, only the right thing and delivered as soon as the patient needs it, creating the best possible outcome for every patient” (White & Griffith, 2019). According to White & Griffith (2019) and Academy of Medicine (2000) there are six criteria for excellence in patient care: safe, effective, and patient centered, timely, efficient, equitable. These six criteria are targets for making sure the patient has the best outcome. Care should be safe where there are no accidents or errors (Academy of Medicine, 2000). Care should be effective where the patients’ diseases and conditions are fully and accurately diagnosed (Academy of Medicine, 2000). Care should be patient centered where the patient’s needs and goals are established with the patient and the family (Academy of Medicine, 2000). Care should be timely without avoidable delay (Academy of Medicine, 2000). Care is efficient and the previous four criteria are achieved at minimum cost (Academy of Medicine, 2000). Care is equitable which means the first five criteria are achieved regardless of patient’s national or ethnic background, geographical location, age, gender or income (Academy of Medicine, 2000). 2. According to Young & Smith (2022), in the article Standards and Evaluation of Healthcare Quality, Safety, and Person-Centered Care , it talks about the factors that contribute to healthcare quality, tools used in delivering quality in other industries that are not regularly used by healthcare professionals and healthcare managers, the bodies that regulate and promote quality, as well as the tools for achieving consensus in healthcare teams. Young & Smith (2022) state that healthcare quality includes all factors contributing to patients’ healthcare status and their acceptance of the care received which includes hard, factual outcomes, such as mortality and morbidity complications, and soft outcomes such as patient experience. Many bodies regulate or promote quality in healthcare. National standards in the U.S. are determined by the federal legislature, the federal courts, the President, and the organizations that report to these entities (primarily the President) which include the US Department of Health and Human Services (DHHS) that also include the Food and Drug Administration (FDA), the Centers for Disease
Control (CDC), the Centers for Medicare & Medicaid Services and the Agency of Healthcare Research and Quality (AHRQ) (Young & Smith, 2022). Independent agencies such as the Joint Commission (TJC) also create and oversee HCO quality of Standards (Young & Smith, 2022). 5. “Excellence in Population Health” is also measured and benchmarked, but the measures are of population, not patients (White & Griffith, 2019). Population Health is measured by the incidence and prevalence of disease, disability, or premature loss of life (White & Griffith, 2019). Excellence in Population Health can be assumed to have the same six criteria as Excellence in Patient Care.
