ONCOLOGY FOLLOWUP HPIP NOTE Patient Name: PEACE, Samuel Tyler Referring Physician: Jennifer Hutto, MD Date of Visit: April 19. 2023 Sex/Age: M/59 DOB: 01/11/1964 Patient ID: 000157 HISTORY: Mr. Peace, 59-years-old, is hold position of tumor bed of high grade, soft tissue sarcoma of posterolateral chest wall. He has done quite well at home. His appetite is good. He is having no pain or no problems with functions of either extremity. He has not yet returned to work. Physical Examination: On exam his scar is well healed, nontender, animal bite. There are no superficial masses or ecchymoses. IMPRESSION: Final pathology shows residual sarcoma. Extensive fibrosis. PLAN: I explained to Mr. Peace that the margins of position are clear. The discussion of whether he would be prudent to adjuvant chemotherapy. Given that a relatively small primary tumor and complete excision, I would find it difficult to justify conventional chemotherapy for soft tissue sarcoma. He is going to follow up again with Dr. Hutto in her clinic to discuss chemotherapy. I would like to see the patient again with a repeat X-Ray. My recommendation would be with monthly follow up with repeat chest X- Ray and physical examination. I am releasing Mr. Peace back to work on light duty next month. He is not to lift more than 10 lbs.
