Every type of hospital is eligible for accreditation from the Joint Commission. A hospital seeking
accreditation must have “10 inpatients served, with one active at the time of survey. A hospital that is
seeking Medicare Certification and is new to The Joint Commission must have two active inpatient case
at the time of survey.” (

The process for accreditation involves reviewing the standards of the Joint Commission, and figuring out
what the facility is lacking and needs to improve in order to meet them. Once the improvements have
been implemented, contact the Joint Commission, and schedule a survey. The application fee requires a
$1700 deposit. When the survey is scheduled, decide who will accompany the surveyor. Have at least
four months of data readily available. Prepare a timeline for the improvements that need to be made by
the time of the survey. Participate with the surveyor at the time of the survey. Complete post survey
follow up activities. A decision will be rendered within two months of the survey date. Joint Commission
accreditation for a hospital lasts for three years. (

The hospital must have a medical record service that has administrative responsibility for medical
records. A medical record must be maintained for every individual evaluated or treated in the hospital.
There are four standards that need to be met:
Organization and staffing
: The medical record service must be organized in accordance with the scope
and complexity of the services provided. The hospital must hire enough people to ensure that records
are completed, filed, and retrieved on time. (42 CFR § 482.24 - condition of participation: Medical Record

Form and retention of record
: Each inpatient and outpatient must have a medical record kept by the
hospital. Medical records must be accurate, timely completed, correctly maintained, and retained, and
easily accessible. The hospital must implement an author identification and record-keeping system that
assures the integrity of the authentication and the security of all record entries. (42 CFR § 482.24 -

Content of record
: The medical record must include facts to support the diagnosis, justify admission and
continuing hospitalization, and document the patient's progress and response to drugs and services. (42

Electronic notifications
: if the hospital uses electronic notification, it must be secure and comply with
regulations (HIPAA). Electronic transmission of patient records must also comply with the patient’s
preferred settings for privacy. The hospital must do everything reasonably possible to make sure that the
system sends the notifications to all relevant post-acute care service providers and suppliers. (42 CFR §
