selected for this case study is L. Davis who was born December 31, 1973 and is now the age of 48. The client is an African American woman. Davis is currently at Reality Community Health Services Inc., where they have provided a woman’s
group for her to reside. The agency provides group therapy services for Davis and 6 other women. The client graduated from Northwestern Senior High School and then attended Miami Dade to further her education. Davis then transferred from Miami-
Dade College to Florida Memorial University where she dropped out with a few credit hours remaining. I

ve had the pleasure of meeting this client while interning at this agency for my hours of supervised field experience. Davis has been diagnosed with bipolar and schizophrenic after she was released from jail in 2003. The client did self-admission into Jackson memorial hospital where she was diagnosis and referred to Reality Community Health Services Inc. Davis losing her kids was a traumatic experience to her. Davis now has the tools from the agency to help her move up in her situation. This particular client had the department of children’s and families take her kids due to the abuse between her and her husband. The client had 4 kids in total 3 girls and 1boy. Since the client lost her kids she has been on a down spiral causing an ongoing cycle of TGK to homelessness to Jackson Memorial Hospital to Reality Community Health Services Inc. since 2017. I understand Davis way to cope with losing her kids to DCF. We have her being homeless and watching her kids grow up in someone else care for years and could not do anything about it do to her situation. Unfortunately caused a domino effect of her going
Dominique Dennis SOW 412 4/9/2021 to jail for stealing or assault to a police officer. The stress and anger of not being able to get out of her situation caused her to lash out unlawfully. Davis being diagnosed with bipolar and schizophrenic helped her understand why she was copping with this differently from most and cause her to start taking her medication. Once Davis takes this medication she has clear thinking and is able to understand the necessary steps she has to take to get out of this predicament and to successfully build a better bond with her now adult kids. With this client I have helped her create a action plan to reach her goal of starting a housing program, which she has done before. This client needs her birth certificate and social security so that she can qualify for the housing program. We talked about the funds to accomplish this goal and transportation which luckily she can have the items mailed to her. So far the client has obtained her social security as of April 2021 and is currently waiting on her birth certificate to be mailed.
