Describe one example for each of the levels of prevention that pertain to Ralph’s case study.
a.
Ralph demonstrated primary prevention by abstaining from smoking and walking
every morning before going to work after being diagnosed with moderate
hypertension and mild coronary artery disease (CAD). Secondary prevention
would be when Ralph's doctor provided him medicine to treat his hypertension
and CAD to keep it from further deteriorating. When Ralph's wife was diagnosed
with breast cancer, hospice offered home health services to aid Ralph in providing
her daily care as an example of tertiary prevention.

Using the risk assessment tool provided in the chapter discuss an estimate of Ralphs
current risk for developing a serious cardiac condition in the future.
a.
Although risk assessments are not used to decide on medical care, they should be
used as warnings, and in Ralph's case, he needs to be more aware of his quality of
life and what his physical well-being requires. Considering the risk assessment
tool, Ralph appears to be at high risk of developing a worse cardiac ailment than
the one he already has. He has a number of controllable risk factors combined
with a few non-controllable risk factors that are accumulating and will deteriorate
his health over time. He has gotten melancholy after the loss of his wife, which
has resulted in his lack of appetite, being concerned about his future and who
would care for him, and now having a persistent cough. He is also overweight,
which increases his risk of high blood pressure, diabetes, and heart disease. The
non-controllable risk factors include being inactive due to the snow, but also
having discomfort in his left knee from an old sports injury.

As a health navigation professional describe how you would define your role to Ralph
when he calls your office
a.
When Ralph contacts my office, I would inform him that it is my responsibility to
make his life easy in all relevant ways. I'm here to provide him with all the
knowledge he needs to enhance his daily life. Organizing and arranging
appointments, locating support groups, corresponding on his behalf with medical
personnel, assisting with financial resources, transportation, hospice, etc. I'm here
to support him as he navigates a new stage of life, to help him navigate the
healthcare system and locate the best options for him, and to assist him in
overcoming any barriers he might encounter that prevent him from accessing
treatment or care.

List four questions that you could ask Dr. Schwartz if you were invited to accompany
Ralph to his medical appointment.
a.
Is his persistent cough an indication of a worsening disease or just a side effect of
the temperature change?
b.
Do you think Ralph's current mental condition has an effect on his physical
health, and if so, are there any mental health services that can be provided?
c.
Is it feasible to begin arranging physical therapy to ease his knee discomfort and
assist him in losing weight?
d.
How frequently must he visit your office and will he need start taking any new
medications to aid with his health based on his existing medical state and the
chance that he may develop a more serious disease?
