1.
Why are general health, medical history and wellness
questions part of a health history questionnaire?
General health, medical history and wellness questions are a part of a
health history questionnaire because it is likely that athletes could
have underlying health conditions that could affect nutrition status and
therefore the dietary recommendations that a sports nutritionist will
recommend.
2.
List at least five questions you could ask an athlete to clarify
why he or she has sought a nutrition consultation.
1.
What are you dietary and fitness goals?
2.
Are you currently experiencing any health issues that are
affecting your athletic performance?
3.
Are there any specific foods that you are interested in
adding to your diet?
4.
Do you plan to make any changes to your current exercise
routine?
5.
Do you want to change your weight or body composition?
3. Explain how you would know an athlete is in the preparation
stage of changing nutrition behaviors related to eating a lower-fat
diet.
3.
Give an example of how a nutrition professional might
breech confidentiality of health or nutrition information
shared in a private consultation with an athlete.
HIPPA law are in place to protect patients private information from
being shared with anyone that does not have the right or need to know
that information. A nutrition professional could breech confidentiality of
protected health information if they share the name of an athlete that
they provide care for and the person they share that with then leaks
the information to the media or some other source.
