HCPCS II Codes 1 Ms. Lates came into the Emergency Department at Global Care Medical Center with complaints of progressive weakness and instability for the past few days. In the past, she was tentatively diagnosed with Parkinson’s Disease. She was then directly admitted to the hospital for observation to rule out parkinsonism and cerebrovascular accident. Her primary diagnosis at the time of discharge was Parkinsonism. After thoroughly reviewing Ms. Lates' chart, I found several procedures that should be coded. These include documentation stating the patient has a past diagnosis of Parkinson’s, admission for observation, electrocardiogram, tap water enema, ambulance transport to home, and Meals on Wheels services. HCPCS II codes are used when filing health plan claims for nonphysician services, supplies, medications (typically medications that are administered other than by mouth, e.g., IV medicines), transportation services, and equipment (Green, 2022). The main terms for these procedures are Parkinson’s, documentation; observation; electrocardiogram; enema; transportation; and meals, home delivered. In the Discharge Summary, it states that Ms. Lates was previously diagnosed with Parkinson’s, which could be a contributing factor to her current visit to the hospital. When I searched for “Parkinson’s” in SelectCoder, multiple codes were listed for documentation that states the patient was diagnosed with Parkinson’s. For this reason, I determined that the appropriate code G2151, Documentation stating patient has a diagnosis of a degenerative neurological condition such as ALS, MS, or Parkinson's diagnosed at any time before or during the episode of care. I also determined that the patient will be directly admitted from the ED (Emergency Department) to the hospital for observation from the History & Physical as well as in the Progress notes section of the chart. I searched “observation” in SelectCoder under HCPCS
HCPCS II Codes 1 category. I determined the HCPCS II code from the list to be G0397, Direct admission of patient for hospital observation care. While she was in the ED, Ms. Lates received an ECG (electrocardiogram). I would assign the code G0405, Electrocardiogram, routine ECG with 12 leads; interpretation and report only, performed as a screening for the initial preventive physical examination. When I reviewed the Doctor’s orders, I found several orders that require HCPCS II codes. These included an order for a tap water enema, ambulance services to get the patient home, and arranging meals on wheels services for Ms. Lates. To verify that the enema was given to the patient, I went through the Nurses’ Notes and determined that it was given to the patient. When I searched “enema” in SelectCoder, the code I deemed most appropriate is A4458, Enema bag with tubing, reusable. This will code for the supplies used for administering the enema. I searched for “transportation” in SelectCoder and several options came up. The code I chose for the ambulance transport is A0080, Non-emergency transportation, per mile - vehicle provided by volunteer (individual or organization), with no vested interest. I believe this code to be most appropriate as it is a non-emergent ambulance transport facilitated by the hospital. I also searched for the code to use for the Meals on Wheels program. S5170, Home delivered meals, including preparation; per meal, is the recommended HCPCS II code for this service (Codify, 2023). The codes I will assign the record are G0397, Direct admission of patient for hospital observation care; G0493, Skilled services of a registered nurse (RN) for the observation and assessment of the patient's condition, each 15 minutes (the change in the patient's condition requires skilled nursing personnel to identify and evaluate the patient's need for possible modification of treatment in the home health or hospice setting).
