Joseph, a 65-year-old Native American anthropology professor who recently retired, is the focus of this treatment plan. He's experiencing anxiety and distress related to retirement and social interactions. As Joseph's counselor, I am 44 years old woman with a minority background who has been through Social Anxiety Disorder. Contextual Assessment (C): Joseph's cultural identity as a Native American of Cherokee descent is an important contextual factor. He is married to Nancy for 46 years, and they have two adult daughters. Joseph had a successful career in academia and was dedicated to his work. Declining vision and hearing are impacting his self-esteem and exacerbating his anxiety. He feels trapped and isolated, preferring to stay at home to avoid social situations. Assessment and Diagnosis (A): - Joseph is experiencing anxiety, hopelessness, and social isolation. - Possible diagnosis includes Social Anxiety Disorder. - The Beck Anxiety Inventory is a potential assessment tool that counselors can find useful when evaluating and diagnosing anxiety disorders. This questionnaire helps counselors understand anxiety symptoms better and can be used before and during treatment to check progress.
Necessary Level of Care (N): - Initial assessment suggests that Joseph would benefit from outpatient individual therapy. Strength-Based Lens (S): Joseph's strengths include his dedication to his career, close family ties, and willingness to seek help at his wife's suggestion. Treatment Approach (T): - Cognitive-behavioral techniques will help Joseph identify and challenge negative thought patterns. - Exposure therapy will gradually expose him to social situations. - Cognitive Behavioral Group Therapy: A 12-session program that combines cognitive restructuring with exposure to Joseph's feared situations. Aim and Objectives of Treatment (A) (1-month objectives): - Reduce Joseph's anxiety and hopelessness. - Increase his ability to engage in extracurricular activities with his wife and friends. - Improve his self-esteem and sense of safety in social situations.
