Milestone One 2 Gap “The provision of mental health care at CBOCs is comparable to that at VAMC clinics, although differences in patient access to offsite care indicate that full equity was not achieved at the time of the survey” (Kirchner, et al., 2008). In recent years, I have seen Veterans, such as my Spouse, who have had the same appointments scheduled at different places. Interestingly, Kirchner and other note that the same type of mental healthcare is available to Veterans at both the VA medical hospitals and the VA’s network of outpatient-based community clinics. The VA has also recently started allowing Veterans to schedule these appointments with private providers, external to the VA, if wait times are too long for the Veteran to wait. Even though today there are issues with the delivery of mental health services to veterans, the problem is much deeper and more closely attributed to the culture within the Military. Military members are trained to be selfless, courageous, and think of everything but themselves. From day one, it is drilled into their heads that the mission comes first, above everything else. This leads to a culture that makes it acceptable for not only the military member to misalign priorities when it comes to healthcare in general, but in particular, mental health carries a stigma in the military. “Veterans may be wary of being open with their problems out of fear for their careers, or, if they’re a single parent, worrying their children might be taken away” (Dave, 2021). The veteran community is most likely one of the most under-informed minority groups in America. The Dept. of Veterans Affairs simply does not have the staff, funding, or network infrastructure to support the collaboration and education that the Veteran community needs. History
Milestone One 3 The history of Veterans access to mental health care has really exploded in the past few decades, for multiple reasons. “According to the National Alliance on Mental Illness (NAMI), upward of 1 in 5 veterans experience post-traumatic stress disorder (PTSD) each year — compared with just 1 in 25 civilians who experience PTSD. Additionally, suicide rates of military service members and veterans reached an all-time high in 2020, with deaths by suicide increasing by 25% by the end of that year” (Virgina Commonwealth University, 2022). Veterans are 5 times more likely to suffer from Post Traumatic Stress Disorder Vs. their civilian counterparts. Although, PTSD seems like a newer mental health condition, the problem has existed for centuries. Both civilians and military members have suffered from PTSD, whether it be a traumatic experience from a car accident when we were kids, to the veteran that stepped on an IED, or watched a brother/sister in arms perish. Those images, memories, and feelings never quite leave you, and impacts human kind across the board. From PTSD being labeled as shell shock, gross stress reaction, or acute mania, it wasn’t until 1980 when the American Psychiatric Association actually added Post Traumatic Stress Disorder to the 3 rd edition of their diagnostic and Statistical Manual of mental Disorders (Friedman, 2020). There are confirmed reports dating back to Ancient Egypt of laborers reporting similar symptoms of PTSD, which, indicates it’s been a misunderstood condition for an incredibly long time. The sheer influx of active-duty service members, post 9-11, has been the cause for the VA and private practitioners to become more acquainted with and informed of PTSD. Specifically, the rigors of war are those that aren’t encountered frequently, and it creates a new and challenging environment for practitioners to navigate. Socioeconomic Background
