Hi Matthew and Rebecca, welcome! I’ve put together a presentation for you today to help explain what the intake process
will look like. 2
Starting with the 13614-C form, usually completed by the taxpayer
before arriving at the VITA site is an important first step to filing your return. This form gives the preparer the information necessary to complete the 1040 form. Since this form is directly used to help complete your tax return the information given must be honest and complete to the best of your knowledge. You and the preparer are both responsible for making sure that your tax return information is filed completely and accurately. 3
The intake process will begin with verifying the taxpayer’s identity through a photo ID. You will also need to bring the social security cards of everyone listed on the return. These documents used to verify your identity are very important to not only prevent fraud but to prevent your return from being delayed if any of your information is recorded inaccurately or incorrectly. 4
The filing status is determined on your marriage status. Single and
married filing jointly are straight forward. Married filing separately is also as it sounds, you could be legally married but separated from your spouse and wanting to file your taxes separately or decide not to file jointly for other reasons. Head of Household is a single person with qualifying dependents and a Qualifying Surviving Spouse would be someone whose spouse has passed away within the last two years and has a dependent. Photo ID is required for the taxpayer for all filing statuses and the
spouse for Married Filing Jointly. 5
Earned income is money you get from receiving wages, salaries tips or self-employment income. Unearned income is more passive and can be received through interest income, dividends, prizes and awards, and gambling winnings to name a few. Documents for proof of income are required for either type. As seen here from your 13416-C form, you received income in the form of a W-2 and passive income of scholarships, retirement payments and social security benefits. 6
The income checked off on the 13614-C form directly matches up to the data in your 1040. W-2 payments, pensions and annuities from retirement benefits, your social security benefits on line 6, and the 1098-T gave information for the American Opportunity tax credit on line 29. 7
This is also true for the expenses you checked off and the proof of expenses documents you brought in. These tie directly to your 1040 and in the adjustment to income portion of the Schedule 1. 8
And again, for life events it’s important to know these and fill out this portion of the 13614-C form, as with the others, honestly and completely because it will affect your 1040. Events in your life could play a big role in changing the income or deductions on your
tax return in your favor, or not but the truth is still just as important, nonetheless. 9
All your income, expenses and life event information determine which forms need to be filed out by your preparer. This will also allow us to identify the certification level a preparer needs to do your return and check if your situation is within the scope of service for the program. After this a preparer will be assigned to
you and you can move on to the interview process. 10
Form 15080 is a consent form to disclose your tax return information to the VITA tax preparation sites. If signed this allows information from the current year’s tax return to be disclosed to the site software, which will then pre-fill in basic information on next year’s form. This basic information will only be carried into the next year and does not allow the site to access any prior years returns. This makes completing next year’s return go faster and easier, so I do encourage you to sign it, and thank you for your time today.
