Wk10assignBakerK6101 By Day 3 Post a response to the following: Explain how you might evaluate client progress Client’s progress can be evaluated by updating the treatment plan regularly. Clinician usually completes an updated treatment every six months to address the goals and see if the client is improving or if the goals are completed. The goals should be address during every session in some form but the official review of the goals will state the progress the client is making. Determining when a client is ready to terminate services. A client is ready for terminate services when all of their goals have been met. Client goals should reflect the things they are wanting to achieve in most cases. The one expectation is when the client is court ordered for treatment. Once the client and/or clinician feels the goals have been achieved the client can be discharged from services. I most cases clients are given the option to come back if they have other concerns arrive. Describe a situation when a professional relationship may end before the client achieves their goals. A professional relationship may end before the client achieves their goals if the client moves out of the area. Post social service programs offer services to certain areas and once a client moves out that area they are referred to a similar organization in the area they are moving to. Clients are usually linked to these services with a warm transfer. This meaning the transfer is set up in advance for continuity of care. Describe one potential positive feeling that you, as the social worker, might feel regarding a planned termination and an unplanned termination. Social worker may feel positive about the termination of services because the client usually when services are terminated it is because the goals have been met. It feels rewarding to know that you have helped someone achieve their goals and have a better quality of life in some form. Describe one potential positive feeling an unplanned termination An unplanned termination can feel positive as well because they may have been discharged from services because the client has gained more support from family or friends and no longer need assistance. Client could also be
moving out the area due to better resources and support in another area. This can feel like a positive move for the client, which intern makes the Social worker feel good about the termination of services. Describe one potential negative feeling that you, as the social worker, might feel regarding a planned termination and an unplanned termination . A negative feeling a social worker may feel when completing a planned discharge would be the feeling of losing someone in your life that you have grown to know personally. The relationship can be professional but social workers have feelings revolving around the positive relationship they have gained with the person or family. There may be some doubts about the client being able to continue with the progress they have made so far without those reminders and follow ups that are provided from the social worker. Describe one potential positive and one potential negative feeling a client might feel regarding a planned termination of the therapeutic relationship. Client may experience positive feelings towards being terminated from the services they are being provided because this means in most cases they have met their goals. Client may feel they are independent and ready to handle life’s challenges without assistance. The negative feels they may have is not having that support system in place when things in life may not go as planned. Client may also feel a since of loss due the relationship that was built with them and the social worker. Describe one potential positive and one potential negative feeling of an unplanned termination of a therapeutic relationship. Client may feel a positive way about an unplanned terminations because they may feel they never needed the help in the beginning. The client may have been court ordered to receive services and this unplanned termination may be because they are now off of their court order and no longer have to follow up with treatment. feelings of abandonment, grief, and loss supportive of their reactions and helped them cope with their feelings therapist was required to meet with his or her supervisor to plan for the initial session allowed them to process the ending better capacity to talk to the previous counselor before the transfer process participant described how dealing the loss was challenging empathy, genuineness, and responsibility that facilitated the process of the transfer
