Communication Safety Leadership Medication Administration 5. Intervention/Teaching completed by the clinical instructor with the student: Concern was identified & reviewed with instructor & student. Potential/Actual impact of concern on patient/client reviewed with student. Student removed from patient care & sent home. As required by AHS (Alberta Health Services)/alternate agency an RLS (Reporting & Learning System for Patient Safety) incident form was completed (if applicable). Student instructed to review pertinent information related to areas of concern. This can include but is not limited to theory content or lab skills. Student instructed to book time in open lab to practice skill of concern. Student provided contact information for Student Services # 780-644-6130 for additional support.
