Report
Burnout in the Healthcare System Shumaila Rahman Capella University NHS400: Developing a Healthcare Perspective Regina Mirabella January 9, 2024
Burnout in the practice of medicine is a complex and pervasive issue with widespread implications for both physicians and patients. This paper provides an overview of the topic’s professional relevance to curbing burnout for physicians, which will require actions at the system level ,  as well as a greater degree of collaboration among clinics, health systems, insurers, government, and medical societies. For the article selection process, the library database resources were used for credibility purposes, and a summarized annotated bibliography of four peer-reviewed articles was provided to highlight the issues. The paper concludes with lessons learned from the annotated bibliography on this topic. Professional Relevance Increasing administrative burdens, inadequate support in practices and health systems, and miscommunication with patients are all playing a key role in increasing stress leading to burnout thus decreasing the quality of healthcare provided to patients. An example of burnout is when I forgot to open a drain after attaching a collecting bottle to the drainpipe in a mastectomy patient during surgical rotation because I was the only doctor on the floor managing 40 inpatients while following orders and doing paperwork for patients admitted via outpatient department. Article Selection Process To provide relevant literature on the healthcare issue of burnout, proper article selection is vital. I used summon and databases of Capella library such as ProQuest, PubMed, and JAMA Network. I used key terms such as burnout and healthcare challenge to narrow my search. to ensure their current relevance use a filter to see articles published within the past five years.
Assessing Credibility The credibility of selecting articles is of vital importance. Criteria for credibility assessment include examining the author's qualifications, the source's reputation, and the presence of a peer- review process. Furthermore, articles without potential biases embedded in them are reputable. Credible journals and academic institutions are reliable sources of evidence-based information. Annotated Bibliography Agarwal, S. D., Pabo, E., Rozenblum, R., & Sherritt, K. M. (2020). Professional Dissonance and Burnout in Primary Care. JAMA Internal Medicine , 180 (3), 395-401. https://doi.org/10.1001/jamainternmed.2019.6326 Burnout negatively affects physician health, productivity, and patient care. A total of 26 PCPs (21 physicians, 3 nurse practitioners, and 2 physician assistants; 21 [81%] women) from 10 primary care clinics participated. They had a mean (SD) of 19.4 (9.5) years of clinical experience. Six common themes emerged from PCPs’ experiences with burnout: 3 external contributing factors and 3 internal manifestations. Participants described their workloads as excessively heavy, increasingly involving less “doctor” work and more “office” work and reflecting unreasonable expectations. They felt demoralized by work conditions, undervalued by local institutions and the health care system, and conflicted in their daily work. In sharing their perspectives on factors contributing to burnout, frontline PCPs interviewed during this study described dissonance between their professional values and the realities of primary care practice, an authority-responsibility mismatch, and a sense of undervaluation. Practitioners also identified possible

solutions institutions might consider investing in to resolve professional dissonance, reduce burnout rates, and improve professional fulfillment.  Jacobs, Linda M,B.S., R.R.T.-N.P.S., Nawaz, Muhammad K,M.P.H., M.B.B.S., Hood, Joyce L, MPH, R.N., C.O.H.N.-S., & Bae, S., PhD. Burnout Among Workers in a Pediatric Health Care System. Workplace Health & Safety, 60(8), 335-44. https://doi.org/10.3928/21650799-20120726-03 Occupational stress among healthcare workers is exhibited not only in traditional ways such as absenteeism, presenteeism, excessive turnover, and illness, but also in non-traditional ways such as decreased patient satisfaction, healthcare errors, and decreased morale in the work unit. Lim, H. Y., & Ooi, S. B. S. (2023). A systems thinking approach to tackle the national challenge of burnout among healthcare workers. Singapore Medical Journal. https://doi.org/10.4103/singaporemedj.SMJ-2021-317 Given the advancements in medical training, a disproportionate increase in workload might be the main reason for the mismatch between work demand and the ability to cope. Examining the trends that will continue to shape health care in the next few decades, such as the aging population, increasing burden of chronic disease, rising patient expectations, newer models of care, as well as tightening medical data security, we can expect the demands on HCWs to continue to rise to unsustainable levels unless drastic actions are taken. Acknowledging that workload is indeed the major contributing factor to burnout is a key step in our quest towards reducing burnout. Other than the increase in the total amount of workload, we additionally theorize that observable shifts like work and work-related stress have
an even greater impact on burnout trends. The cognitive burden on HCWs has also increased tremendously. Today, junior doctors are expected to be up to date with the latest evidence and be able to quote journal articles and cite statistics from a vast body of scientific research that has exploded in the past decade. The individual is expected to recall vast amounts of medical information, protocols, and pathways, to synthesize an acceptable management plan. There are also more mandatory professional training courses. Matthias, W. (2022). Physician burnout undermines safe healthcare. BMJ: British Medical Journal (Online); London , 378 . https://doi.org/10.1136/bmj.o2157 A substantial proportion of healthcare professionals report symptoms of burnout. Research into the negative ramifications of physician burnout is abundant, with key insights already established long before the onset and ongoing burden of the COVID-19 pandemic. Burnout was also associated with doubled odds of patient safety incidents, low levels of professionalism, and significant decreases in patient satisfaction. Hodkinson and colleagues’ research adds to growing evidence that the poor mental health of healthcare providers jeopardizes the quality and safety of patient care. Reflections on Developing the Annotated Bibliography The process of creating an annotated bibliography along with assessing the credibility of selected literature made me learn how important is this issue of burnout to be addressed and resolved to sustain the healthcare system with positive outcomes on both patient and care provider well-being.
