Analyze the increased complexity of care among older adults.
Caring for older adults can be a complex and challenging process. Nurses must possess many skills to care for seniors with chronic illnesses and mental health issues, including excellent communication abilities to provide prompt and effective care. Nurses must actively involve patients and their families in all care plans; the care process can be complicated due to medical,
social, and psychological components, making it less efficient. The elderly often experience multiple coexisting diseases, making it difficult to treat them effectively.
Compare care models for nursing practice specific to the older adult.
Elderly patients may be on a more difficult drug regimen than younger patients and suffer from cognitive impairment or other age-related problems. To make the best decisions for them, rehabilitation personnel need access to information about the elderly's specific condition and needs. The Geriatric Assessment can provide this information. The IMPACT program is a care model that builds upon evidence-based treatment for depression and incorporates approaches to managing other chronic diseases, such as diabetes. (Unutzer, et al., 2002) Those approaches include collaboration among patients, primary care providers, and specialists. The development
of a personalized treatment plan, proactive follow-up and monitoring, and protocols for stepped care (Unutzer et al., 2002). The IMPACT program has been successful according to a variety of measures. Patients had higher rates of depression treatment, for example, and more significant reductions in depressive symptoms than nonparticipants (Unutzer et al., 2002). The Indiana University School of Medicine, the Indiana University Center for Aging Research, and the Regenstrief Institute developed the GRACE model. The GRACE program offers comprehensive senior care in the comfort of patient's homes. A team comprising a nurse practitioner and a social worker conducts an initial assessment at the patient's home and provides follow-up care at least once a month, either in person or over the phone. In addition, the team provides home visits to patients who have been to the emergency department or hospital recently. They work with the patient's primary care physician to create a care plan that includes protocols for treating 12 specific geriatric conditions. The interdisciplinary GRACE team
meets weekly to discuss the patient's progress with a minor team. (Institute of Medicine, 2008)
Identify local, state, and national resources facilitating safe and effective
care transitions for older adults.
One resource that caught my attention was ACL. The organization advocates for the well-being of older adults, people with disabilities, families, and caregivers at the federal level. They allocate funds to states and community-based programs that provide various services and support. Additionally, they invest in training, education, research, and innovation. The program aims to assist in health and wellness, protect individuals' rights, prevent abuse, promote
consumer control, strengthen community-based organizations, and fund research, among other things.
Incorporate professional values, attitudes, and expectations regarding ageism when caring for older adults.
When caring for an elderly person, involving them in their care is essential if they are capable. They should have the ability to make decisions regarding their treatment. Medical professionals must maintain transparent communication with them and explain things in a way they can understand.
Outline the importance of advocating for older adults in management of their care.
As a nurse, you need to speak on behalf of your patient. If you feel like they are not getting the proper treatment or they are being abused, you, as a nurse, need to speak up. Some older clients don’t speak up as they feel like they are a burden to their families and will abandon them. They must get the proper care just like everyone else.
