Daniel Wilfong Supply Chain Survey  Describe your professional experience with a Supply Chain organization or department. This could include a professional relationship with a SC organization in a non-Supply Chain role (finance, marketing, sales, HR, IT, etc.) or a role you held in a SC organization. What functions and organizations were involved and briefly describe the experience? All of my supply chain experience would came my role in accounting. I have worked closely with supply chain in managing inventory and distribution. Currently my role is managing the inventory and distribution accounting for my current employer. I have also worked for logistics companies so have some experience in that stage of supply chain.  Answer the applicable question below o In addition, if you have held a role in a SC organization, identify the position, organization and company or  o In addition, if you have NOT held a role in a SC organization, include this point in your response. I have not held a role in a SC organization. I have worked closely with the SC department not held a role in SC.  Identify the company where you are currently employed. List the supply chain departments/functions in the company. You may need to initiate a conversation with a supply chain team member to accomplish this. My current employer is Save A Lot. Save A Lot is a wholesale grocery supplier. Save A Lot has a Supply Chain department and these different functions as the distribution centers - distribution, inventory, logistics, warehousing, shipping, receiving.  Do you have a contact in a Supply Chain organization in your company or another company? (In this course, the research project requires you to engage with a person in a Supply Chain organization.)

o If so, identify the company and the individual’s name, title and department. Discuss briefly the person’s responsibilities. Yes, Zach Thompson Supply Chain Director. Zach is responsibly for managing the supply chain for the cooler and frozen grocery. His team manages the ordering and demand and ensuring the products get to the stores. o If not, will you be able to establish a Supply Chain contact in a company for this course work? Identify the potential contact (company, individual's name, title and department) and briefly discuss the person’s responsibilities. NA
