KNES 315 Name(s): Janese Jones Assignment 7 30 points Due Sunday, Dec 6 by 11:59pm Instructions If you attend synchronous class, you will be randomly assigned into groups to work on this assignment during class. If you choose not to work with a group, you can complete the assignment independently. If you do not to attend class, you are required to complete this assignment independently. o This assignment is calculated into your overall grade. Assignments should be uploaded to Blackboard Sunday, Dec 6 by 11:59pm o If you complete assignment in groups, only one assignment needs to be submitted per group. Be sure to include first and last names of all group members. You made it to the end of the semester, congratulations. This final assignment has four parts to it. My goal is for this assignment is two-fold. First, I want to focus on this final unit, Upper Extremity Injuries and Concussion. Second, I want to get a little feedback from you regarding the content of this course. Part 1: Upper Extremity Injury Research and find a video of a recent (within the last 2 months) professional athlete who suffered from a traumatic upper extremity injury. Watch the video and complete the following. 1. Provide the link to the video. Sam Darnold (BROKEN SHOULDER) Injury | NFL 2020 - YouTube 2. Provide a brief (< 5 sentences) explanation of the injury scenario. Who the athlete is, the game environment, what happened, what injury the athlete suffered. Sam Darnold is a professional NFL starting Quarter Back for Newyork Jets. The game took place on October 1 in Newyork City. During the game against the Denver broncos Donlad is seen picked up and slammed backwards onto his right shoulder. Sam Donald suffered an AC Joint Sprain. 3. Describe the initial care the occurred for this athlete. If the video does not show the initial care, describe what care SHOULD have been performed? The video does not show any initial care for this athlete after the injury occurred. However, some players can play through this injury but in the case with him being the QB he needs to get the full assessment before returning to the field. First, the athlete training staff will ask him pain related questions. Next, the athlete training staff will apply ice and place him in a sling, while sending him in the back to get an x-ray to rule out a fracture. Last, a complete physical exam such as point tenderness to the AC joint will be once the other injuries are ruled out to determine it is an AC Joint Spain. 4. If your example/video presents an injury that was discussed in class, compare and contrast this video to the information provided in lecture. For example, was the mechanism of injury the same? From what you could tell, were the signs and symptoms the same (what signs and symptoms do you suspect if you are unable to actually see anything from the video)?
Or 4. If your example/video presents an injury that was NOT discussed in class, do a quick search for information on the injury video you found. Provide general information on this injury. Such as, what structures are involved? Typical mechanism of injury? Typical signs and symptoms. An AC Joint Sprain was discussed in class. Just as we learned in the class this player suffered this AC Joint Sprain due to directly being slammed onto that right shoulder. Sam Darnold showed some of the signs and symptoms we learned in class he immediately started holding his shoulder and showing in his face the level of pain he was in. From the video you could tell he was in pain as his arm swung while moving. I cannot see deformity or point of tenderness from the video. However, I suspect as the athletic trainers examined his shoulder whenever they touch that area it was tender allowing them to pinpoint his injury before and after evaluation of possible fractures. Part 2: Upper Extremity Complete the following questions based upon the provided scenarios. 1. While playing ultimate Frisbee during a physical education class, a 10th grade student collided with another player as he was reaching for the Frisbee. He fell on an outstretched arm and his now on the ground in obvious pain. a. If the injury is a dislocated elbow (ulnar dislocation), what responses would you expect during the history component of an assessment? What’s wrong? What hurts? “I hurt my elbow” Can you explain what happened? (establish mechanism of injury? “I felt like my elbow extended more than normal (hyperextended)” Where is the pain? Can you point to a spot where it hurts the most? “My elbow really hurts…it’s everywhere, not just one spot” Do you have pain anywhere else in your body? “Not really…my arm and hand hurt, but my elbow is really bad “ Can you describe the pain? “It was sharp at first, not everything just hurts” What is the level of pain on a scale from 1 – 10? “It’s definitely a 10!” Did you hear anything when it happened? “I thought I heard a cracking sound.” Did you feel any unusual sensations (e.g., tearing; tingling; numbing; cracking) when it happened? “It felt like something snapped or cracked.” b. While completing the history component of the assessment, you observed the area. If
the injury was a dislocated elbow (ulnar dislocation), what would the observation reveal? There would already be visible swelling to the area and an obvious deformity. The student would likely by holding the arm in flexion, with the forearm appearing shortened. A total loss of function would be apparent. 2. During a high school basketball game, a 16 year old player on your team attempts to throw an overhead pass. In attempting to steal the ball, an opponent hits the player’s arm while it is in an overhead position and forces it into extension. The player is instantly in pain and unable to continue. a. What questions would you ask as part of the history component of an assessment of this condition? The following questions should be asked: What is wrong? What hurts? (establish the injured area) Can you explain what happened? (establish mechanism of injury) Where is the pain? Can you point to a spot where it hurts the most? Do you have pain anywhere else in your body? Can you describe the pain? What is the level of pain on a scale from 1 –10? Did you hear anything when it happened? Did you feel any unusual sensations (e.g., tearing; tingling; numbing; cracking) when it happened? Have you ever had this injury before? b. While performing the history, you observe a sharp contour on the affected shoulder. You also note that the player is holding the involved arm in slight abduction and external rotation. It appears that the player is apprehensive about moving the arm. You suspect an anterior glenohumeral dislocation. Would you perform the palpation component of an assessment for this injury? If yes, explain the palpation process. If no, explain the reason for your decision. Palpation should not be performed. Sufficient information can be obtained from the history and observation to determine the need for continued assessment and/or management of the condition. Part 3: Concussion Complete the following questions based upon the provided scenarios. 1. You are a high school field hockey coach. A field hockey player has been hit in the head with a ball. When you reach the athlete, you find her to be conscious, but dazed. a. Explain your initial on-the-field assessment of this potential head injury. Explain the reason for your response. baseline. This information can then later signify signs of improvement or deterioration. b. Throughout the assessment, you should be observing for potential signs of concussion. Identify the specific signs that should be observed while the athlete is still seated on the field. Periods of loss of consciousness, presence of a vacant stare, disorientation, observable in coordination with balance or dizziness, slurred or incoherent speech, and emotions out of proportion . c. Determining the presence of certain symptoms is an important part of the assessment of a head injury. Identify the symptoms that should be noted. (i.e., as volunteered by the athlete or asked by you). Headache, nausea/vomiting, dizziness, feeling of confusion, fleeing sluggish, sensitivity to light or noise, blurred vision, balance problems, inability to concentrate, and inability to recall things. d. What techniques can be used to gather a history of this injury and test the mental status of the individual? The techniques are determining the level of disorientation: asking the individual questions about the situation, assessing the ability to concentrate by reciting numbers, and assess memory by repeating a series of words. e. In continuing the assessment, you determine that it is appropriate to assess for coordination and balance. How can you accomplish this task? The finger to nose test: touching index finger to nose of one hand and then alternating. Gait: Observing the individual walk in a straight line Romberg test: Individual stands still with eyes closed to observing swaying. f. The athlete was seen by a physician and diagnosed with a concussion. She is not permitted to participate in sports/ physical activity until the physician re-evaluates her condition. Is this individual susceptible to postconcussion syndrome? What are the signs and symptoms of postconcussion syndrome? Any individual is susceptible of the syndrome after diagnosed with a concussion. The syndrome is characterized by persistent headaches, blurred vision, vertigo, memory loss, irritability, decreased attention span, and inability to concentrate . 2. You are working as a football coach at a middle school. A mother of a football player comes up to you and says she is confused about what a mild traumatic brain injury is and why you are holding her son out who just suffered a MTBI yesterday. Provide your response to this mother. Be sure to explain what a mild traumatic brain injury is and why it is not recommended for her son to play football/sports with lingering signs/symptoms. Good afternoon, Your son unfortunately suffered a MTBI yesterday. MTBI is a brain injury at the time your so was injured he showed a little confusion and dazed when asked his name, birth date and what year it was. Even though he was able to answer those questions due to his struggle with answering and the impact of the injury he must sit out until a signs and symptom are gone. Your son suffered a mild concussion if he
would have change in his mental status or lost consciousness it would be considered a concussion. Sometimes concussion signs and symptoms occur days later, with that said he is out right now to ensure his safety and full recovery. I will recommend taking him to get a physical exam, let them know our athlete trainers suspect he suffered a MTBI. Please feel free to contact me if you have nay other concerns. Part 4: Semester feedback This is the first time I have taught this course. I would appreciate some general feedback about the course (and the virtual format). If you are completing this assignment in groups, I would appreciate everyone in the group answering the following questions. How do you feel the speed of the course was? I feel like the speed of this course was fine, I love the flexibility. 2. How do you feel regarding the amount of information providing in this course? Do you think the information provided will be useful to you in the future? I think the information will be useful as a future physical educator and basketball coach. I will now know my responsibilities and limitations when a child or student get injured. Furthermore, I know how to treat injuries when they occur before the paramedics comes or further medical treatment becomes available. 3. Would you have preferred if this course was taught synchronously? No, again I like being able to watch the videos on my own. I struggle with paying attention when logged on in class with everyone doing things in the background or constantly moving. I recommend keeping how it was this semester I wish my other classes was own pace, 4. Is there anything you would recommend I change for the next time I teach this course? Whether in person or virtually? I would recommend study sheets for the class information such as a read along activity with the chapters to help study. This will help students retain the information better, it has been a struggle to retain the information because I am not using it day to day .
