Pharmacology Scenario 1: Junetta Cooper Guided Reflection Questions Opening Questions How did the simulated experience of Junetta Cooper’s case make you feel? I definitely felt very nervous. Even though it was a simulation, it felt so real, and I couldn’t help imagining what it would be like in a real life situation. I panicked a bit in the beginning, because I suddenly felt this huge amount of responsibility for this patient, and the realization that this person’s well-being was dependent upon my critical thinking and decision making skills dawned on me. However, once I was able to get into the flow, I was able to calm down and better apply my nursing knowledge base to the clinical situation. Did you have sufficient information to care for the patient effectively? I had sufficient information to care for the patient effectively because I was able to get information from the SBAR regarding Mrs. Cooper’s current status and care needs, and I was able to refer to the Provider’s orders, the MAR, and the patient’s medical records to guide my plan of care. Tell me the actions you felt went well within this scenario. I feel I was able to incorporate patient safety measures, pain assessment, and patient education well in this scenario. I washed my hands, checked patient identification, and always assessed all aspects of the patient’s pain first before administering medication. I also educated the patient about the medications I was giving her, including the proper use and storage of them. Reflecting on Junetta Cooper’s case, were there any actions you would do differently if you were to repeat this scenario? If so, how would your patient care change? I would definitely streamline my questioning process a little more so it’s easier for me to remember all the information. The first time I completed the simulation, I was very disorganized, and I would forget to ask important questions. I would also time my patient education better next time so that it coincides with when I am administering her medications. I identified her priority problems as uncontrolled chest pain (angina), high blood pressure, coronary artery disease, and a suspected myocardial infarction. PCC Identify Junetta Cooper’s risk factors specific to cardiovascular disease. Patient is over the age of 65, is African American, and has a 20 year history of hypertension. S/PCC What potential problem could arise from sublingual nitroglycerin therapy, and what teaching should be included specific to this potential problem? Patients can experience severe headache following administration of sublingual nitroglycerin, so it is important to inform the patient that it is a common side effect. Acetaminophen can be used for the headache. PCC/EBP Discuss the potential synergistic effect(s) of Junetta Cooper’s medication, and identify the potential side effect this could cause. It is important to monitor blood pressure and heart rate because the combination of nitroglycerin and morphine can increase risk of hypotension. T&C What other interprofessional team members should be involved in Junetta Cooper’s care? Other interprofessional team members involved include a primary care physician, pharmacist, cardiologist, and nutritionist. Concluding Questions Describe how you would apply the knowledge and skills that you obtained in Junetta Cooper’s case to an actual patient care situation. I will always remember the importance of maintaining patient safety, including hand hygiene, checking patient identification, asking about allergies, and checking the MAR before administering any medications.
