1-2 Discussion: Implementing Telemedicine Systems After reviewing the health IT strategic planning chapter, one best practice that stood out was conducting post-discharge follow-up telehealth visits. As I consider implementing this practice, collaborating with stakeholders and developing a strategic project plan will be necessary. The best practice of conducting follow-up telehealth visits with patients after hospital discharge would help with collaboration and project management as we strategize implementation. Coordinating these additional virtual visits would require involvement from multiple stakeholders - care teams, hospital administration, patients, and their caregivers. As the project lead, I would work with each group to develop policies and workflows for setting up and conducting the visits. For example, I collaborated with hospital case managers and physicians to identify eligible patients and get their consent for follow-up calls. I was partnering with clinical and IT staff to integrate telehealth into discharge processes and ensure proper documentation, educating patients on the benefits and addressing barriers to participation.
