Capstone Project Final Written Report Outline
I.
Title Page
The title page should be APA formatted, and include your name, project title, course information, and date. Review and update any changes from the Title Page
that was developed in MPH/680.
II.
Abstract
Review and update any changes from the Abstract that was developed in MPH/680. The abstract should be located at the top of your document and limited to 350-words. The abstract summarizes the Project Final Report and closely resembles the Logic Model
. The following should succinctly be addressed in your abstract (e.g., no more than a few sentences per item):
Introduction
containing the problem under investigation and the purpose of your project (including your measurable goal)
Methods
including what you will do, how you will collect your data, as well as a description of the sample or participants and the sample size
Results
including your implementation findings (for the Proposal, you will state the hypothesis or what you believe will happen as a result of your measurable goal)
Implications
that discuss why the study is (or will be) important, and the importance or application of the results or findings

Acknowledgements
This is optional but should be included here if desired.
IV.
Introduction to the Problem/Issue Introduce the Final Report by discussing the origins or Background and Need of the health problem or issue as well as the role of the agency, and the impact of the problem or issue in the community. This section also includes the implementation strategy and expected outcomes of the Capstone Project. Refine and finalize the Introduction
that was developed in MPH/680, and confirm the following are included:
a. Explain
the problem and populations impacted.
b. Describe
how this problem is influenced by larger systems using a Social-Ecologic Model Approach.
c.
Describe
the agency you are working with and the services they currently provide.
d. Discuss
stakeholder input related to identifying the problem and need for your project.
e. Describe
, at a high level, the project to be implemented and expected outcomes.
f.
Outline
other data and/or evidence that justify the need for this project.
V.
Literature Review
This is an overview of peer reviewed research that addresses the problem or issue. Refine and finalize the Literature Review
developed in MPH/680 and ensure that you have addressed the following:
a.
Compared
the approaches or solutions to the problem in each article.
1.
Are the findings and program plan implementations offered by each article similar or dissimilar? 2.
If dissimilar, do the findings and program plan implementations contradict one another?
b.
Critiqued
the articles. 1.
Which program plan implementations seem most effective? Why? 2.
How were the programs evaluated for sustainability?
c.
Described
existing community programs that address the health issue. 1.
How successful are they in treating the health problem? 2.
Does the community embrace their missions? 3.
Do the target populations see them as relevant or helpful?
d.
If no local programs exist, determined the reasons. 1.
For example, are there socioeconomic, physical, political, or environmental reasons no local programs have been established? 2.
If so, can you provide any recommendations that might mitigate those factors?
VI.
Methods
Describe the completed Capstone Project implementation plans, including project activities, data collections plans, data analysis, and the project timeline. Build, expand, and transition the similar section from MPH/680 that described what methods would
be used into the methodology discussion of what took place
during the implementation and evaluation of the Capstone Project. (Note: It may be helpful to incorporate MPH/680 course materials related to Logic Models and stated Implementation and Evaluation plans). a.
Project Purpose
Using the Problem Statement
from MPH/680, describe or list the purpose of the project by including goals and measurable learning objectives. Briefly explain the public health problem you are addressing and anticipated outcomes
. This should be its own section and may be in outline format.
b.
Target Population Analysis
Explain the health needs of the population served by reviewing and updating any changes from the Target Population Analysis
that was developed in MPH/680. This section describes an overview of the target population impacted by the health problem or issue. Ensure the
following information is included: demographics, gender, age, ethnicity, educational levels, socioeconomic status, etc. Outline data and/or evidence that demonstrates the health needs of the population served. Describe gaps in knowledge (data, information) and how you will address the gaps in knowledge.
c.
Completed Project Activities
Describe the project methods/intervention/activities that were implemented during MPH/690. (
Hint: Build, expand, and transition the Proposed Implementation section from MPH/680
). The following should be addressed in your explanation:
1.
Describe
the assessment procedures or activities that were utilized during project implementation.
2.
Explain
the research strategies or approaches that were used for the Capstone Project.
d.
Completed Data Analysis
Discuss how the data were analyzed during MPH/690 and include data reporting methods. (
Hint: Build, expand, and transition the Proposed Evaluation section from MPH/680
). The following should be discussed in your analysis:
1.
Outline
the data and/or evidence that justified the need for this health project or initiative.
Describe the metrics collected.
Explain trends demonstrated in the data.
Describe the results achieved.
VII.
Project Timeline
Describe the timeline for the project that took place during both MPH/680 and MPH/690. Include and reference the Action Plan and Timeline and/or Logic Model. The Logic Model can be included in this section or in the Appendices, however it should be explained in this section.
VIII.
Results
A written presentation of the project implementation results. Include a discussion of the data, as well as corresponding visuals (e.g., charts, tables, etc.), that address the following:
1.
Description of the project outcomes.
2.
Discussion of the implications for practice.
IX.
Discussion, Recommendations, and Conclusion
As a result of the project implementation, discuss any recommendations. Describe how the project met the mission and/or needs of the agency and the population they serve. Include any issues that generated during

A one-page graphical depiction of your logic model, that illustrates the following:
Assessment procedures
Necessary resources or ‘inputs’
Short, intermediate, and long-term community impact
Evaluation procedures
